One patient record that survives the second visit
Duplicate patient records are the quiet tax on a busy lab. Two entries for the same person means split history, a report attached to the wrong visit, and a conversation you cannot win when the patient asks about last month.

A UHID generated, not typed
Every patient gets a lab-unique identifier at registration, built from the date and a daily sequence. It is generated by the system, so two staff registering at the same counter cannot produce the same number.

Search by phone number, name or UHID from the registration screen. The lookup runs before you commit, which is the only point at which a duplicate is cheap to avoid.
Age and sex are working data, not decoration
Both are captured at registration because both decide which reference range applies to a result. A paediatric patient recorded as an adult gets flagged against the wrong range, and the report reads as normal when it is not.
Delivery preferences on the record
Whether this patient wants their report on WhatsApp, by email, by SMS or on paper is stored with them, so it does not have to be asked again.
Referral attached at the point of registration
The referring doctor, hospital, TPA or collection centre is recorded on the visit, which is what makes the revenue-by-source picture and the commission calculation possible later.
